Volunteer TB Risk Assessment Clinic Schedule
Only TB Risk Assessment Questionnaires can be completed and
reviewed at the District Office on the days specified below.
Risk Assessment Clinics are being held in the DO East Building in the small conference room from 2:00 – 5:30pm on the following dates:

If the registered nurse is unable to sign the TB Risk Assessment
based on your answers, you will have to complete a TB skin test at the medical
provider of your choice at your own expense.
TB Risk Assessments and TB skin test results are good for four years.
